In order to send you Disclosure and Barring Service (DBS) application forms we will need the following:

CMAA Members & Affiliates.
Please contact the CMAA office (see below for contact details) with your membership number and the forms will be sent to you within 7 days. If you are not sure whether you are an affiliate or not please include the name of your National Body when you contact us.

Non CMAA Members
We will require the following:
1. Your full name
2. Your full address (including postcode)
3. The name of your Company or Martial Arts Group (not Club)
4. *Proof of your grade(s)
5. *Proof of your Instructor qualification OR proof or running a club while a member of a National UK based Martial Arts body
6. A contact telephone number
*These details are required due to the fact the the DBS certificate you will receive will carry the Cobra Martial Arts Association name along with your role title which could lead to the wrong assumption that we as a national group have vetted your martial arts back ground and qualifications.
